    Surgery

    Every patient with a liver tumor should be evaluated for a resection (surgical removal of a section of an organ). It is the only chance for cure. Removing the tumor will rid the body of the cancer and also prevent further spread to other regions. The goal of liver resection is to completely remove the tumor and the appropriate surrounding liver tissue without leaving any tumor behind.

    Alternative Treatment Options
    • Radiofrequency Ablation (RFA)

      This utilises microwaves to generate thermal energy and heat within the target tissue, using needles placed into the lesion under image guidance. It is usually used for irresectable lesions and be applied percutaneously (through the skin), laparoscopically or during laparoscopy.

    • Transarterial Chemoembolisation

      This involves infusion of emulsion of chemotherapy agent mixed with lipoidal into the feeding artery of the tumour, followed by embolisation. This is usually for large iresectable tumoours limited to the liver with no involvement of the portal vein.

    • Chemotherapy

      Chemotherapy uses drugs to treat the cancer. These drugs work by interfering with the growth of cancer cells and lends to cell death. Effectiveness of treatment is evaluated regularly with physical exams, blood tests and x-rays. A drug is considered effective if it causes the cancer cells to shrink, to stop growing, or to die.

    • Liver transplantation

      Liver transplantation has become an accepted treatment for patients with end-stage (advanced) liver disease of various types. Moreover, liver transplantation is the best option for patients with tumors that are less than 5cm in size who also have signs of liver failure.

    AboutDr Tay Khoon Hean

    Dr Tay Khoon Hean is an internationally-trained liver surgeon who specialises in liver cancer treatment.

    Dr Tay studied Bachelor of Medicine and Bachelor of Surgery at the National University of Singapore obtaining MBBS in 1998. He was trained under the tutelage of two world renowned surgeons.

    Dr Tay was awarded a Travelling Fellowship Scholarship from the Royal College of Surgeons in Glasgow for advanced laparoscopic surgery to understudy Prof Alfred Cushieri in Ninewells Hospitals, Scotland. He then spent another year to train for hepatobiliary surgery at the Pontchaillou Hospital, University de Rennes in France. His areas of expertise are in advanced laparoscopic surgery (minimally invasive) and surgery of the liver, bile duct, gall bladder and pancreas. For liver resection, he is utilising the French techniques and makes use of the PPD or pylorus preserving pancreatoduodenectomy when it comes to pancreas cancers.

    Dr Tay Khoon Hean continues to serve various medical committees both locally and internationally. He is also a visiting doctor in National Cancer Centre and Singapore General Hospital.

    What To Expect On Your Initial Visit For Liver Surgery/ Cancer Consultation

    The initial consultation with the surgeon should be a pleasant one and normally starts to any physician visit where you will undergo a thorough medical test. Your doctor would want to know your personal health history or if there was an incidence of cancer in your family. Make the most of your appointment by coming in prepared and by answering the questions thoroughly. It is important if you know the medications that you are taking or if there are symptoms that you experienced: persistent abdominal pain, nausea and vomiting, etc. Have you done a recent CT scan or MRI? You may bring a copy of the imaging report as it will be helpful to your surgeon.

    Meeting with the Liver Surgeon

    The doctor does a complete physical examination and asks about the patient’s personal and family medical history. Other tests may be ordered, such as CT scans, x-rays that give detailed pictures of a cross-section of the liver if you have not underwent a recent one. You can also reschedule these tests or procedure in a time and date that is convenient to you though some patients are encouraged for an urgent diagnosis. No surgery is performed without any health study as a basis or the actual images. By the end of your visit, your doctor will only recommend that is best for your care or there are other treatment options aside from the preoperative testing.

    • What is Liver Cancer?

      Liver cancer (hepatocellular carcinoma) is a cancer arising from the liver. It is also known as primary liver cancer or hepatoma. The majority of primary liver cancers (over 90 to 95%) arises from liver cells, which make up 80% of liver tissue, and is called hepatocellular cancer or carcinoma.

      Metastatic liver disease (cancer) or secondary liver cancer, is cancer that has spread to the liver, having originated in other organs such as the colon, stomach, pancreas, breast, and lung. Thus, the term liver cancer actually can refer to either metastatic liver cancer or hepatocellular cancer.

    • What are the symptoms of Liver Cancer?

      Abdominal pain is the most common symptom of liver cancer and usually signifies a very large tumour or widespread involvement of the liver. Additionally, unexplained weight loss or unexplained fevers are warning signs of liver cancer in patients with cirrhosis. The sudden appearance of abdominal fluid and swelling and jaundice (yellow colour of the skin), may suggest the possibility of liver cancer. On physical examination an enlarged, sometimes tender, liver is the most common finding.
